Key takeaways
- The medical plan is set by a physician or a licensed centre. Get it in writing, with expected length of stay and follow-up needs.
- Medical care, a rest stay and a leisure trip are three different things with different contracts and expectations.
- Check step-free access, a working lift, quiet, a kitchen, ventilation and a safe bathroom in the actual unit. Do not assume.
- Measure the real route to the specialised centre. The north suits some plans and not others.
- Azhianeh provides no medical services or referrals. Stays are discussed per project file.
What does health tourism mean here, and what does this guide cover?
This guide covers only accommodation and planning. It says nothing about treatments, doctors, methods or results, and it cannot.
People often mix three things. Medical care is delivered by a physician or licensed centre. A wellness or rest stay is a calm place to be, with no treatment on site. Leisure travel is sightseeing and visiting. All three can sit in one journey, yet each has its own responsible party, contract and expectation.
If you do not separate them, two mistakes follow. You treat a guesthouse as if it were a treatment centre, or you buy a medical plan as if it were a holiday package.
Who decides the treatment plan, and what should you get in writing?
Only the treating physician or a licensed centre decides the plan. A hotel, travel adviser or intermediary does not replace them.
Before you book a stay, ask the physician or centre for these items in writing:
- A summary of the proposed plan and the approximate start date
- The expected length of stay, and a note that it may change
- Follow-up needs after discharge or after the course ends, and where they happen
- Mobility, diet and care limits that a companion should know about
Without this page, a long booking is a gamble. You might reserve a short stay and the plan runs far longer, or the other way round. If the physician cannot give an exact length, ask them to write that, and book on flexible terms.
How can you check that a provider is licensed?
Ask the provider for the licence, its scope, a written price list and the complaint route, then confirm the licence independently with the body that issued it.
Write four questions and ask for written answers. Which body issued your licence, and what does it cover? What is the official price list, and how can it change? If something goes wrong, who receives a complaint and how? Who is the responsible physician or technical lead?
If someone avoids showing a licence, answers only with spoken promises or lists of past clients, or pushes you to decide today, treat that as a serious warning sign. This guide names no centre and recommends none. The choice is yours and your physician's.
What does a companion need from a stay?
A companion usually needs comfort, quiet and simple access, and each point should be checked in the actual unit, not only in a brochure.
The list below is a prompt for checking. The patient's physician and family define the real needs:
- A step-free route from the car park or entrance to the front door, and thresholds that will not stop a wheelchair or walking aid
- A working lift. Ask what happens during a power cut and how fast repairs are made
- Real quiet at rest hours. Listen for neighbours, the lift and the plant rooms
- A kitchen that can handle a special diet, with a fridge and room for simple cooking
- Fresh air and ventilation: a window that opens, no damp smell
- A safe bathroom: non-slip floor, grab rails or a seat, and room for assistance
- A simple route to a pharmacy and clinic, checked on a real map at a real hour
If something you need is missing, it is better to learn it early. A message with photos or a short video of the same unit often settles it. Do not rely on what is only said.
Why might the north suit some rest plans and not others?
The north may suit someone whose physician has cleared travel and who mainly wants calm, green surroundings and a slower pace. It may not suit someone who must reach a specialised centre often.
The Hyrcanian forests are on the UNESCO World Heritage List, and many families like the green setting and different air of the Caspian side for a rest period. That is a personal preference, not a medical claim. We say nothing about the effect of climate or nature on any condition.
The other side is distance. If the plan needs frequent visits to one centre, measure the real travel time on that exact route, including holiday traffic and road conditions, and ask the physician whether that distance is acceptable. If not, a stay close to the centre may be the better choice.
Who should you ask about insurance, documents and visas?
Ask only official bodies: your insurer, the embassy or consulate, or an authorised agency. Rules differ by nationality, type of trip and day.
Ask the insurer whether your travel policy covers a journey made for treatment or follow-up. Many policies carry exclusions, so read the wording. Ask the embassy or consulate which visa category fits the trip and which documents, including medical papers, are required.
Banking, flights and sanctions conditions also change. An answer that was right last month may not be right today. We give no legal or consular advice and promise no visa or residence.
How do you protect health data and agree payment terms?
Share the minimum, and only with someone who needs it for their job. Treat a medical report like a banking password.
A guesthouse or agency rarely needs a diagnosis to take a booking. Tell them practical needs only, such as "we need a lift and a safe bathroom". If anyone asks for medical records, ask why, how long they keep them and who sees them.
Avoid sending files through open messaging groups, and carry a paper copy yourself. If you send a document, ask the other side to confirm in writing that they deleted it when the work is done.
Every amount, payment date and refund condition belongs in one written, signed text. A spoken promise or a phone call is not enough.
For the stay, make clear what the price includes, what is charged separately, what happens if the medical plan ends earlier or later, and what is returned, and by when, if you cancel or move dates. Avoid paying in full before you have seen the terms, and keep proof of how you paid.
Treatment fees follow the same rule. They come from the centre, with a written price list, and should not be bundled with the accommodation. We do not comment on the cost or quality of care in Iran.
What is the pre-booking checklist?
Use this table before paying. Ask each question and compare the reply with what a good answer looks like.
If the reply is vague, write it down and ask again. Vagueness costs less now than after you arrive.
| Question | What a good answer looks like |
|---|---|
| What is the medical plan and expected stay? | A written note from the physician or centre, dated, saying the length may change |
| Which body licensed the centre? | The body and scope are named and can be confirmed with the issuing authority |
| Where is the price written? | A written list with change conditions and separate charges |
| Who receives a complaint? | A named contact and route, no evasion |
| Is the unit step-free with a lift? | Photos or video of that unit and a clear answer on power cuts and repairs |
| Do the bathroom and kitchen fit our needs? | Specific answers on floor, rails, space and kitchen equipment |
| How long is the real route to the centre and a pharmacy? | A time on the real route, allowing for traffic and season |
| Does travel insurance cover this? | The insurer's written confirmation or its list of exclusions |
| What if the plan changes? | Cancellation and refund terms, written and signed |
| What do you do with my health data? | Only practical needs requested, and storage explained |
How does a developer design for long stays?
For a long stay, good design is a set of small details felt every day: lift, thresholds, sound, air and upkeep.
The lift needs usable dimensions and a maintenance plan. Thresholds should avoid sudden level changes. Sound insulation between homes and from the plant matters, and we wrote more in acoustic comfort in a coastal apartment. Ventilation has to account for northern humidity.
The other half is operations: who services the lift, where a fault report goes, how cleaning and safety are run. That is covered in building operations and hotel-style services. A good building without good operations is only a promise.
What can Azhianeh say, and what can it not say?
We provide no medical services, make no referrals and give no opinion on treatment. What we can do is discuss the stay.
Any unit or project that might suit a longer stay is discussed in that project's file: plans, routes, lift, operating model and what is still undecided. For Rokan Sense in Mahmoudabad, which is under construction, we say nothing about services or prices, and all project images are concept images. If you are weighing a family base for repeat visits, see the second home guide. For overseas readers asking about property rules, start with the foreign investor guide.
Azhianeh is a Tehran-based architecture and development practice working in Tehran and Mazandaran, founded by Dr Hassan Taheri and Dr Shokoufeh Zaheri.
Frequently asked questions
What does health tourism in northern Iran mean on this page?
It means a trip planned around treatment, recovery or rest, and we cover only the accommodation and companion side. A physician or licensed centre decides the medical plan. Azhianeh is not a medical provider and gives no view on the method, quality or outcome of any treatment.
What should I get from the physician before booking a stay?
A written note showing the proposed plan, the expected length of stay, follow-up needs and any care limits. If the length is uncertain, ask them to say so in writing and book on flexible terms. Without it, you cannot judge how long you will stay or what it may cost.
How do I check that a medical provider is licensed?
Ask the provider for its licence, scope, written price list and complaint route, then confirm the licence with the issuing body yourself. Evasive answers, spoken-only promises or pressure to decide quickly are warning signs. We name and recommend no centre.
What does a companion need from an apartment or villa?
Usually a step-free route, a working lift, quiet, a usable kitchen, good ventilation, a safe bathroom and an easy route to a pharmacy. Check each item in the actual unit through photos, video or a visit. The patient's physician and family define the real needs.
Will travel insurance cover a trip made for treatment?
That depends on the insurer and the policy wording, and many policies exclude it. Ask before buying and keep the answer in writing. For visas and documents, ask only the embassy, consulate or an authorised agency, since rules differ by nationality and by day.
Is the north suitable for every recovery plan?
No. The green setting and slower pace appeal to some rest plans, but if you must reach a specialised centre often, distance matters. Measure the real travel time on the actual route and ask the physician whether it is acceptable for you.
If you need a calm family stay
